TRAINING: BACHELOR'S DEGREE IN SOCIAL WORK FROM AN ACCREDITED COLLEGE OR UNIVERSITY. SUBSTITUTION: BACHELOR'S DEGREE IN SOCIOLOGY, PSYCHOLOGY, COUNSELING, CRIMINAL JUSTICE, BEHAVIORAL SCIENCE, INTERPERSONAL COMMUNICATIONS, HUMAN SERVICES, EDUCATION, SPECIAL EDUCATION, ELEMENTARY EDUCATION, OR SECONDARY EDUCATION FROM AN ACCREDITED COLLEGE OR UNIVERSITY MAY BE SUBSTITUTED FOR THE DEGREE IN SOCIAL WORK. OR CURRENT WEST VIRGINIA SOCIAL WORK LICENSE SPECIAL REQUIREMENT: ELIGIBLE FOR TEMPORARY SOCIAL WORK LICENSE OR LICENSED AS A SOCIAL WORKER, GRADUATE SOCIAL WORKER, OR CERTIFIED SOCIAL WORKER BY THE WEST VIRGINIA BOARD OF SOCIAL WORK EXAMINERS. Job Description

THE INCUMBENT WILL WORK ON A TEAM WITH CHILDREN WITH SPECIAL HEALTH CARE NEEDS NURSE TO PROVIDE SERVICES FOR PROGRAM APPLICANTS LOCATED STATEWIDE. INCUMBENT MUST HAVE A THOROUGH KNOWLEDGE OF SOCIAL THEORY, PRINCIPLES, AND PRACTICES AND MUST LEARN AND FOLLOW CSHCN POLICY AND PROCEDURES. THE INCUMBENT WILL ASSIST IN COMPLETION OF CARE COORDINATION ASSESSMENTS FOR CHILDREN FROM BIRTH TO AGE 21 YEARS, WHO HAVE SPECIAL HEALTH CARE NEEDS. THE INCUMBENT WILL ASSIST IN ARRANGING AND AUTHORIZING MEDICAL CARE EVALUATION, AND SECURING MEDICAL REPORTS TO DETERMINE PROGRAM ELIGIBILITY. THE INCUMBENT WILL EVALUATE FINANCIAL RESOURCES TO HEALTH CARE COSTS, AND DETERMINE PROGRAM FINANCIAL ELIGIBILITY. THIS PERSON WILL WORK TO PROVIDE SOCIAL SERVICES COMPONENT, A PART OF THE CARE TEAM CSHCN PERIODIC PEDIATRIC CLINICS AND WILL MEET WITH FAMILIES IN THE CLINIC, IN THE OFFICE AND AT OTHER PREDETERMINED LOCATIONS. THE INCUMBENT MAKES REFERRALS FOR SERVICES AND COORDINATED TRANSITION TO OTHER SERVICES AS APPROPRIATE. STATEWIDE TRAVEL IS REQUIRED. OTHER DUTIES AS ASSIGNED.
