12th Annual Disability Employment Awareness Forum

8:00am – 3:00pm

Pre-Registration Form

CEU’s will be provided for $10.00 ($15.00 at the door)

Lunch will be available at the cost of $5.00 per person

The forum will be held, Tuesday October 27, 2009 at the Raleigh County
Convention Center, Beckley, WV.

Registration and check-in will be at 7:30-8:00 a.m. with the program
starting at 8:00 a.m.

If you do not need CEU’s, but still plan to attend and want a lunch,
please pre-register and enclose $5.00.  
On the day of the forum you will receive your meal ticket at check-in.
