Collaborative Care for Chronic Pain
A workshop for reflection and action by primary care teams
Friday, May 29th
8:30 am – 4:00 pm
Location: Summit Conference Center
Charleston, WV
Join us for a workshop designed to teach primary care providers and
staff about evidence based care for chronic pain. Explore systems of
care that improve patient outcomes, increase safety, and reduce stress
on caregivers. Learn from the experience and knowledge of peers and
experts. Take away concrete action steps for your practice.
Objectives – Upon completion of this program, participants should be
able to:
1. Discuss the collaborative care model for chronic pain.
2. Identify the strengths and weaknesses of chronic pain care in
their own setting.
3. Describe richer, multimodality care plans for persons with
chronic pain.
4. Outline interventions responding to dysfunctional behaviors by
persons with chronic pain.
5. Identify and intervene with organizations and medical providers
who display dysfunctional patterns of chronic pain care.
6. Develop and commit to an action plan to improve the
collaborative care of chronic pain in their clinical setting.
Registration and Fees:
Registration fee is $50.00 and includes administrative costs,
educational materials, lunch, and continuing education fees.
Participants that are members of the West Virginia Primary Care
Association can register for free. Please complete the attached
registration form and fax/email/mail by May 15, 2009.
