SOCIAL WORKER (Full Time) The social worker intervenes with patients who have complex psychosocial needs, require assistance with eligibility determination for social programs and funding sources, and qualify for community assistance from a variety of special funds and agencies. In addition, offers crisis intervention to patients and families with psychosocial needs and collaborates with the patient care team in the development of a discharge plan of care for all patients including high-risk patient population. This role will receive referrals for individuals from at-risk populations from the interdisciplinary team members (including physicians, care managers, staff nurses, and other members of the care team).
Qualifications include:
- Masters Social Work required. Must obtain Licensed Graduate Social Worker or LSW certification in the State of West Virginia by the end of the first six months of employment.
- One to three year's experience preferred / hospital experience strongly preferred. -Experience in psychosocial and therapeutic counseling.
-Possesses excellent interpersonal communication and negotiation skills in interactions with patients, families, physicians, and health care team colleagues.
-Ability to work with people of all social, economic, and cultural backgrounds; flexible, open minded, and adaptable to change.
- Capable of independent judgment and action regarding psychosocial needs of patients.
