Alzheimer's Care Center Program Director
SunBridge Care and Rehabilitation for Salem is a 112-bed center that
offers skilled nursing and rehabilitation therapy services to adults of
all ages and acuity levels. The 50-bed Alzheimer's Care Center is a
secure unit that is licensed by the State to provide services to
individuals with Alzheimer's and related dementias who may benefit from
daily programming specific to their needs.
Position Summary
This position plans, develops, coordinates, and supervises the Special
Treatment Program (STP) and other therapeutic activity programs in
accordance with SunBridge policies and procedures and State and Federal
regulatory requirements. Works collaboratively with the Unit Manager,
Charge Nurse, and/or Director of Nursing Services to oversee and
supervise all aspects of care/programming for the dementia resident.
Participates in IDC and staff meetings. Organize and plan, with the
assistance of Social Services, monthly meetings to address
needs/concerns of residents and their families. Provide education and
training to all staff as regards dementia, activity based programming
and behavior management. Ensures appropriate documentation is initiated
and maintained at preadmission and throughout the resident’s stay.
Works with marketing to provide outreach/networking to the community and
referral sources. Facilitates communication with all other department
heads to ensure coordination of the program.

Professional Experience/Educational Requirements
Associate degree or certification/license in a relevant field in health
care (RN, LPN, LSW, Recreational Therapist, Activity Director).
Minimum 2 years experience in development and facilitation of
dementia-specific activity programs.
Demonstrated knowledge of Alzheimer’s disease and related dementia.
Prior experience working in a long term care/geriatric setting.
Flexibility to work days, weekends and evenings as required.
This position is a full-time, exempt position and considered a part of
the management team and a key team player.
