SOCIAL WORKER

SUMMARY: Provides social work services to maintain and, if possible,
improve the physical, mental and emotional well being of older adults
and disabled adults in our community.
ESSENTIAL DUTIES WILL INCLUDE: Perform intake registration for new
participants to determine eligibility and appropriateness for programs;
conduct initial assessment, develop care plan, initiate ongoing reviews
and refer as necessary to other services the participant or caregiver
might need; assess participants for mental illness, dementia,
alcohol/substance abuse, physical abuse and/or neglect and make
appropriate referrals for follow-up, treatment, or counseling; provide
discharge planning and assist in transition and follow-up for
participants and caregivers when changes make it necessary to consider
alternative living arrangements; provide individual and group counseling
for participants and families; assist the Program Coordinator with
outreach and community organization activities; provide information to
participants, caregivers, staff and the community regarding community
resources/services; coordinate with Information and Assistance and case
management as needed; provide information and referral for persons not
appropriate for adult day care; make follow-up home visits and calls as
necessary to assist and counsel participants and caregivers, advocating
and safeguarding the human and civil rights of the participant; provide
assistance with feeding, mobility, and toileting, and other duties as
assigned. Work collaboratively with RN to assign volunteer Health
Mentors; develop support groups/health promotion classes at the senior
center based on EW participant needs; provide information, referral, and
follow-up regarding community resources; primary responsibility for
ongoing recruitment. Maintain records and prepare and submit timely
reports; other duties as necessary and/or assigned.
The Successful Candidate will have the following qualifications:
● Bachelors Degree in Social Work or related field.
● At least two (2) years’ experience in a human service field,
preferably working with frail or disabled adults and their families or a
Masters Degree
● Willingness to obtain a yearly TB Test, Statement of Good
Health, Food Handlers Permit, Series B Hepatitis test and CPR
Certification.
● Experience with health promotion/disease management preferred.
● Certification, if applicable, preferred.
● Physical ability to assist disabled participants with
transferring and ambulating; must be able to lift at least 50 pounds.
● Computer experience necessary.
● Valid WV State Drivers License

reviewed. Interviews will be scheduled for those applicants whose
qualifications in the areas
listed above most closely correspond to the position’s requirements.
