Loudoun Community Health Center
Position Title: Clinical Social Worker FLSA:
Exempt
Department Head:
      
  Medical Director
Direct Supervisor:
      
     Medical Director
Overall Responsibilities: The provision of mental health
counseling to
clients of the LCHC. The individual will share responsibility for
ensuring
adequate coverage of intakes, emergency and/or crisis interventions.

Manages, develops, and evaluates LCHC Family and Mental Health
Services Program,
Position Requirements:
    1. Master’s degree in Social Work required; licensure as a
Clinical
Social Worker in the Commonwealth of Virginia or license-eligible
upon hire.
Dual credentialed and certified to provide substance abuse counseling
highly
preferred.
    2. Experience working with the population served (e.g., Latino
immigrants/refugees, clients of all ages and with diverse
socioeconomic backgrounds).
   
    3. Cultural competency such that the individual possesses a
sensitivity, understanding and expertise in caring for persons of
diverse cultures.
    4. Outstanding interpersonal and communication skills.
    5. Written and oral fluency in English and Spanish highly
desirable.
    6. Ability to work within a multidisciplinary team.
Description of Duties:
Client Care responsibilities:
    1. Conducts comprehensive bio-psychosocial assessments.
    2. Provides individual, family, and group therapy to clients of
all
ages.
    3. Identifies critical barriers to self-sufficiency and works
with the
LCHC team to overcome these barriers for the client.
    4. Identifies and refers clients to needed resources.
    5. Provides substance abuse counseling as needed and/or referring
out
for these services.
    6. Provides mediation and client advocacy services as well as
educational seminars/workshops to groups of clients and/or staff.
    7. Develops and implements service and care plans tailored to
address
clients’ circumstances.
    8. Maintains client records such that program and regulatory
guidelines
are met.
    9. Reports on a monthly basis client caseload statistics, data
and
outcomes as required by program expectations and grant guidelines.
Program Development and Evaluation:
    1. Develops and updates the Family Services and Mental Health
Program
Manual of Operation, including policies, procedures, record keeping,
and other
requirements. Works to integrate the mental health program into the
primary
care family practice model
    2. Serves as the primary liaison to the clinic staff of the LCHC.
 
Works with the Chief Medical Office and clinic staff on program
goals, services,
referral guidelines, and outcomes.
    3. Designs and implements surveys and focus groups, including
client
satisfaction surveys. Prepares survey reports for distribution to
Board and
grantors. Discusses survey findings with staff and ensures appropriate

follow-up.
    4. Designs, implements, and monitors an outreach program to
promote
family and mental health services and identify hard-to-reach clients.

    5. Oversees program evaluation, data collection and reporting.
Reviews
client evaluation tools periodically and ensures timely implementation
of
client assessments.
    6. Oversees monthly, quarterly, and annual reports on clients
served,
demographics, services provided, client outcomes, and other grant
requirements. Prepares a written analysis of client and program
performance quarterly,
annually, and as required
    7. Ensures program quality assurance, including compliance with
Medicaid, Medicare, and other federal, state, city, and accreditation
and licensing
requirements.
    8. Works with the Director of Development in program development,

reviewing grant applications, ensuring implementation of grants'
commitments, and
provides program performance reports to be used in the preparation of
reports
to grantors and other donors.
Community Mental Health Network Development:
1. Works to develop and coordinate a community safety net for
menatal
health and substance abuse services for the uninsured or underinsured
in
Loudoun County.
2. Identifies, develops and maintains a referral network of
appropriate
community services and serves as the Center’s liaison to and from
these
agencies.
    1. Represents the Family and Mental Health Services Program in
the
community by making community contacts and fostering strong community
relations
and support for our program.
Job responsibilities may vary from those listed above due to the
demands of
the work environment. If these responsibilities are significant and

Working Conditions and Physical Effort:
    * Work will be performed on-site of the Health Center.
    * Limited physical effort is required.
    * There is minimal exposure to physical risks, such as operating

dangerous equipment and chemical hazards.
    * Reasonable accommodations are provided in compliance with the

American with Disabilities Act.
Supervisory Duties: None
Position responsibilities may vary from those listed above due to the
